Narrative

Employee and two co-workers were in the process of lowering a water line pipe to the ground. EE was using a drill steel to release the pipe from the chain when an s-hook failed, letting the pipe fall. The drill steel was knocked out of employee's hand and it whipped around and struck employee on the forehead.
