#5 MINE ACCIDENT-Employee was working under loadout belt lifting a return idler in squatted position and heard a pop and felt sharp pain in right (Employee started losing time again on 3/25/10 due to surgery to remove hardware) (This employee had originally been terminated on 7/17/09. He was later rehired and is courrently an active employee)
