## Narrative

One of the moveable louvered air vents of the RTD was loose and had fallen out during operation. The operator decided to place it back into position. He was reaching in when he came into contact with the fan cage cutting his right middle finger. The fan cage itself is another 5-6 inches past the louvered air vent.
