Narrative

Location B9HG #3 Entry, 6XC. Personnel were shoveling ribs, a piece of rock (approx. 3x16x18"" in 3 pieces) came loose and fell from the roof and rib line. The rock struck the employee on the left foot. Injuries consist of soft tissue tears to the foot, scraps and contusions to the left knee, left neck, shoulder and left hand.
