It was determined by a Medical Physician that an Employee suffered from an occupational illness due to repetitive motion that is performed in his job as a roof bolter. The employee started missing work on 5/6/09 and a determination was made on 7/14/09. Upon the employee's return to work a return to work form will be completed that will reflect total days starting on 5/6/09.
