Narrative

The roof fall occurred in the #4 Mains Section (#9 Block, #3-#4 Crosscut, 3200ft outby the face). The fall measured 25""x16""x8"" (LxWxH). The area was bolted with 6"" Super Twist bolts. No one was injurd as a result of the fall. Ventilation and travelways were not disturbed.
