On June 2, 2009, employee mentioned to his supervisor that he felt pain in his elbow. After extensive medical evaulation, medical professionals determined that the employee has cubital tunnel syndrome. The employee underwent surgery for relief of this chronic condtion on August 21, 2009. Therefore, we are reporting this as an occupational illness.
