Narrative

Employee ran a chain through the tougue at the rear of the bolter and through the tail of the miner, looping it back to hold tension on the miner while trying to get the bolter unstuck. The hook on the chain broke, causing the chain to fly back and strike employee on the back of his right hand and knee.
