Narrative

Has EE climbed down an extension ladder felt sharp pain in his right calf muscle. EE was taken to an urgent care fac, and seen by Dr. EE was prescribed RX and ice, released to full duty, EE had vacation approved 8/24/09-8/28/09. While on vacation EE went to Reservation Clinic and the Dr. placed him on 10 days restricted duty.
