Narrative

FTE was cleaning scrubbers. He climbed the 3' ladder; hit his head on the overhead pipes. He said he was ok. FTE was asked if he wanted to go to clinic, he wanted to wait until Monday. On Sept.14 EHS Manager did a follow up, FTE feeling better. 10/2 FTE wanted to go to clinic, paperwork received on 10/5, with prescription.
