Narrative

Employee was in the process of cleaning out a shelter hole along the track haulage. His left foot was placed on a coal shelf on the mine bottom by the shelter hole. As he reached down to pick up a large piece of coal toward the back of the shelter hole, his left foot slipped causing him to hyper extend his knee. The employee started missing work on 10/10/09 due to this event.
