Narrative

Working in Precip removing packing from draw off valve. While removing 2nd set of packing, packing blew out of valve spraying caustic in employee's face. Diagnosed with chemical conjunctivitis in left eye. Was prescribed eye drops & reststricted to work duties for one day. He returned to work with no restrictions on 10/08/09
