Narrative

Employee was rolling drive belt onto pump sheave and thumb was caught and pinched between the belt and sheave. Employee was not wearing gloves. Diagnosed as a contusion. On 10/15/09, thumb was still painful, x-ray was done on this date and revealed a hairline fracture. Restricted duty
