## Narrative

The employee was replacing liners in the bin.  The liner was hot tacked in place when he struck it with a 4-pound hammer to further position it.  When the 4-pound hammer contacted the liner at a glancing angle instead of straight on, the hot tack broke and was projected behind the safety glassess into the left eye.
