## Narrative

IGNITION: An employee was welding a bar to the rockdust pipe that was hanging down from the mine roof. He had been welding approximately 45 minutes and stopped to eat lunch. Another employee then burned a hole into the pipe and an ignition occurred from a methane bleeder around the pipe. The flame burned 10 to 15 seconds and was put out with 1 fire extinguisher.
