Roof Fall in 4 East occurred above the bolt ankorage in the Intersection of #4 Entry at 4+ 18 in the Last Open Crosscut. The fall was approximately 80' in length and 8' high. This happened between shifts and was found at 12:10 AM by the 3rd shift prep foreman.
