Narrative

Employee hurt his shoulder/neck area on 08/07/2008. Employee was lifting a pump to be installed on a haul truck and felt a pop in his left shoulder. He returned to work on 4/7/09. He re-injured himself on 7/15/09. He did not report until 7/16/09. The incident has been under investigation by GAB. The claim was accepted 11/18/09.
