An unplanned roof fall occurred in the #3 entry at spad #4292. This fall is approx 1800' from active section in the return airway. Area was developed 9/31/09. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 80'l X 5' thick. State and Federal agencies notified immediately.
