An unplanned roof fall occurred in the #1 entry at spad #4264. This fall is approx 1200' from active section in the return airway. Area was developed 8/15/09. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 100'l X 6' thick. State and Federal agencies notified immediately.
