Narrative

The emp. alleges to have contracted CWP while working at this mine. Emp. was diagnosed with the same classification of CWP when he was initially hired with this company. In 2006 he had a NIOSH sponsored chest x-ray which he did not bring to us until November 2009. Although this is being reported now, the emp. had this condition when he came to work for us initially.
