## Narrative

Employee was using a 4 1/2"" grinder with a cutoff disc attached to dress a bolt on a flange. While cutting excess metal from the flange, the cutoff disc jammed or caught on the flange, causing a portion of the cutoff disc to disintegrate and strike the employee causing a laceration to his left leg.
