## Narrative

Employee was dismounting a production truck.  His hand slipped off the handrail, resulting in employee striking his elbow on a step.   He felt a sensation in his right shoulder and didn't want medical attention.  On 1/15/10 complained of discomfort and was evaluated by a physician.  Was put on restrictive duty 1/16/10.
