Narrative

Emp. in lab felt dizzy, burning eyes, heavy chest. Discovered samples of chemical D31-18 were placed in oven prior to his shift which may have been cause. Removed chemicals from oven. Emp. went to company doctor. X-Ray showed Bronchitis, meds prescribed, restrictions ordered.
