Narrative

Employee was installing pin on strut and was using short 4' step ladder. While descending ladder and standing sideways on lower step, he spun to left and with his right foot stepped off ladder and onto a 20 lb. sledge hammer that was standing with handle in vertical position and slightly to the left of the ladder. The employee had been using the hammer and placed it on the ground.
