## Narrative

A roof bolter operator was installing a cable bolt and while he was using a one foot wrench to spin the cable bolt he looked away at his control levers. Meanwhile, the one foot wrench jumbed off of the cable bolt while he was spinning it and raising it to the roof. The cable bolt was in a bind and the recoil caused the bolt to hit the operator in the right forearm.
