## Narrative

Employee was diagnosed with Carpal Tunnel Syndrome.  Per the employee, first noticed symptoms back in Oct of 2009.  She did not report to the Company until January 5, 2010.  Employee was diagnosed on January 25, 2010.  Scheduled for surgery on February 19, 2010.  Employee continued to work with no restrictions through last scheduled shift before surgery.
