EE slipped and fell on the snow, but he continued to work without reporting the incident. He was sent to the ER on 2/16/2010 and was treated for a shoulder contusion and released to full duty. At his follow-up appt. on 2/25/2010, he was prescribed physical therapy thereby changing the incident from 1st aid to medical recordable.
