Narrative

Employee went to check the level in a tank he did not see a broken bag of biocide on top of the tank. When he bent over and opened the tank inspection hole the pressure from inside the tank blew upward blowing biocide into his face. The employee did not go to the doctor until much later and was referred to a specialist on 03/04/2010 and given a prescription medication.
