Narrative

Employee lost his balance while burning bolts on a grate pressure bar and became impaled on a grate bed depth finger. The incident was reported at 8:09 a.m. to USS Management, at 8:35 a.m. to Dave Wallin with MSHA, and at 8:54 a.m. to the MSHA Emergency number by Laurie Potter. The employee was removed from the grate at 8:40 a.m. and from the property at 8:50 a.m.
