## Narrative

During an inspection, a hot spot was discovered by 2 MSHA inspectors & OAEI's plant manager. There were no flames present but steam was rising from a small area on the impoundment. OAEI was not permitted to take any action to remediate the hot spot. Approx 50 minutes later, MSHA issued a ""K"" order. A plan had to be developed before any remediation could begin.
