Narrative

Belt clamps were being used with ""lever style chain hoist"" to pull the belt together. During this process the internal brake of that lever hoist slipped which shifted the pressure to the lever hoist that employee was operating. The sudden shift caused his right little & ring fingers to get caught between the chain of the lever hoist and a trough idler frame.
