Narrative

On 28 Nov 2009 individual heard a pop in his left shoulder while lifting a case of boosters in daybox. During medical exam 29 Nov individual was released to perform regular duties. Arthoscopic surgery on 6 Apr 2010 indicated damage to rotator cuff and bicep. Surgery made this a MSHA reportable incident.
