Slip and Fall in Ice and Snow Conditions on Portable Dock Ramp.  No Lost Time, No Modified Duty. Accident became reportable when found out from corporate office via insurance company that employee had received physical therapy. Records at plant stated physical therapy prescribed by clinic but had not been administered by clinic. *** Medical Treatment Only ***
