Narrative

Employee was washing out under station #2 when a rock approx. 7"" diameter bounced off scalp screen falling approx 20' striking employee on top left side of hard hat. The force drove hard hat down on EE's head causing bump and laceration to left side of head. Upon being examined at ER the Dr. administered 5 staples to laceration on employee's scalp.
