Narrative

Employee was bolting off the miner in 4 West. As he was pushing the roof bolt into the hole, the bolt coupler got stuck on the hole in the channel. The employee reached over to push the bolt free from the channel and it sprung catching his left hand between the bolt and the plank jack.
