Narrative

Employee states while checking a hot roller the shirt sleeve on his left arm got caught by the roller, which resulted in his left arm being pulled into the roller. He sustained two fractures to his left arm which required surgery. Also his shoulder and face were burned while they were up against the roller until the arm was disengaged.
