Narrative

Employee was working at the washer plant & he went to clean mud filters by opening 2 different valves. When attempting to clean the 2nd one he missed the first step of the stairway & cut his forehead at the edge of one of the steps. At the clinic, no concussion or fissure was detected. He got 11 stitches (1.5"" cut at the forehead)& was released for regular duty.
