Narrative

Employee stated that after scaling some rock down with a bar he had pain in his shoulder. He is a 58 yr. old man who has had problems on the contralateral shoulder. Dr. report showes bone spurs, tendinosis and condition present from before claim. This is being turned in as a repetitive degeneratve illness. Employee had shoulder surgery 05/24/2010.
