## Narrative

Employee was assisting another employee to remove a nipple from a 1 inch ball valve. He was using a pipe wrench and as he pulled the pipe wrench toward his body he felt two pops in his shoulder followed by pain. Diagnosed as a dislocated shoulder. Work restrictions applied 5/31/2010.
