Narrative

EE was using a 3 ton chain come-a-long to gather slack chain being generated by the headgate tensioner when the face conveyor flight that was being held in place by the stops broke. This let tension release back into the face conveyor chain, causing the hook on the chain come-a-long to bend & release striking EE on the left lower leg causing injury.
