## Narrative

Climbing down from the grade all machine when his left hand slipped from the hand grip causing him to put all his weight on his right arm which in turn caused a tear to the rotator cuff of his right arm. He continued to work at his regular duties until 6/4/2010 when he was removed for surgery to the right shoulder.
