## Narrative

The man doors on the south skip were not properly secured by an employee at the end of 2nd shift.  When the south skip was taken into the dump scrolls to test safeties, it became stuck on the ""A-Arm"" because of the open doors.  The skip was damaged making it difficult to extract and interfering with the use of the hoisting equipment for several hours.
