A fall of roof was discovered in the 3I panel in the #5 Entry at spad 19+45. The size of the fall was 20ft long x 19ft wide x 6 1/2ft high. This fall was previously reported on e-document #1132712. Per a discussion held with a D10 Roof Control Specialist it was determined that (2) separate roof falls could not be recorded on the same 7000-1.
