## Narrative

Two employees working on seized pump. Employee locked the pump out, unseized it; took off the lock and reset it.  Pump seized up again so the employee instructed his partner to turn the pump back a second time with a 4 foot pipe wrench.  The pump started and the wrench flew out of the partners hand and hit the employee in the right forearm.
