Injured and co-worker were roof bolting a cut in the #3 entry when a piece of roof rock 39"" long x 4"" to 10"" thick x 0"" to 7"" wide broke out between the ATRS and the Safety Post Jack hitting him on the back of the head pushing him face first into the bolting machine causing facial fractures.
