A Fall of roof was discovered in the 1st Northwest Mains in the #1 Entry. The fall extended from x-cut 119 to x-cut 128. This fall was previously reported on e-document 1066177 on 2/24/2010. Per a discussion held with a D10 Roof Control Specialist it was determined that (2) separate roof falls could not be recorded on the same 7000-1 form.
