A fall of roof was discovered in the 1A Submain extending from x-cut 16 to x-cut 20. The fall was approximately 250ft long x 19ft wide x 6 1/2ft high. This fall was previously reported on e-document 1066131. Per a discussion with a D10 Roof Control Specialist it was determined that (2) separate falls could not be reported on the same 7000-1 form.
