A fall of roof was discovered in the 1A Submain at x-cut #8 extending from #4 Entry to #5 Entry. The fall was approximately 120ft long x 19ft wide x 6 1/2ft high. This fall was previously reported on e-document 1066131 on 2/24/2010. Per a discussion with a D10 Roof Control Specialist it was determined that (2) separate roof falls could not be recorded on the same 7000-1 form.
