A fall of roof was discovered in the 1A Submain in the #3 entry from x-cut 5 to x-cut 6 it extended over to #2 entry at x-cut 6 and connected up to x-cut #7. This fall was previously reported on e-document 1066131 on 2/24/2010. Per a discussion with a D10 Roof Control Specialist it was determined that (2) separate roof falls could not be recorded on the same 7000-1 form.
