Narrative

A roof fall occured in the #11 seam in the 1 Main East, #5 entry between crosscuts 11 & 12. The fall measured approximately 25'x12'-15'x3'. The area was supported with 3' fully grouted roof bolts. The fall occurred due to a slip in the mine roof.
